By signing this form ("The Releasee") grant permission and consent to Karla's Klozet ("The Releasor") for the use of following videographer(s) as identified below for a presentation under any legal condition, including but not limited to: publicity, copyright purposes, social media, advertising, and web content.

Description: Website, company use

Payment: I understand that there shall be no additional payments for this session.

Royalties: I understand that there shall be no royalties, fees or other compensation shall become payable to me by reason of such use.

Revocation: I understand that with my authorization below the videographer(s) may never be revoked. We, the Releasor and Releasee, understand and agree to the aforementioned terms and conditions.
